Thank you for scheduling your eye exam with Dr. Hodges.  In order to make the check-in process as efficient as possible and to avoid rescheduling, please follow the checklist below in preparing for your appointment.

1.  Please arrive 10 minutes prior to your appointment time to check in an complete additional paperwork.

 

2.  Please print out the forms on the buttons below, fill out completely and bring them to your appointment.

3.  Please bring your most current insurance card, insurance referral (if required) and your copay.

     * Without a current insurance card, you will not be seen.

     * If your insurance plan requires a referral and you do not have one, you will not be seen.

     * Copay is due at the time of service and cannot be billed.

4.  If a family member has Power of Attorney for your health care, please bring the appropriate paperwork.

5.  Any patient under the age of 18 must be accompanied by an adult.

If for any reason you are unable to keep your appointment, please give the office 48 hours notice in order to avoid a $100 missed appointment fee.  For those who are experiencing cold or flu symptoms, we ask that you reschedule your appointment in order to protect the well-being of our staff and other patients.
 

In the event of inclement weather, missed appointment fees do not apply.
